Documentation Updates - #301

Couple of updates for documentation:

1. A getting started guide to register an application with Twitter.
2. Made the inline documentation easier to find by linking in the sidebar.
3. New example program using the streaming API since there weren't any in the examples folder.
4. Updated the shorten URL documentation per issue URL shortening parameter is not used in api.py, therefore service is not available  #298.
