SUMMARY

The 'Add primary storage' dialog has a duplicated 'Provider' drop-down selector.

STEPS TO REPRODUCE

Go to Infrastructure > Primary Storage and click the 'Add primary storage' button.

EXPECTED RESULTS

There should be only one 'Provider' drop-down selector.

ACTUAL RESULTS

There are two 'Provider' drop-down selectors in the dialog.
